HOW CAN I REGISTER:
Come to our location (early arrival is recommended as we provide service on a first come first served basis). We will register you as you go through the drive-through line or you may complete the Intake Form in advance.
HOW DO I REGISTER IN ADVANCE:
For your convenience and to speed up the process, you may fill out the INTAKE FORM AHEAD OF TIME. Download and print the desired form (English or Spanish) below. Please print legibly when completing the form. Leave blank any areas that are unclear to you. Bring the form to the church ahead of time if possible or the day you come to pick up your food.
REGISTRATION FORMS ARE AVAILABLE AT THE CHURCH AS WELL.
